What is BMR?

BMR, or basal metabolic rate, is the amount of energy the body uses at complete rest to sustain basic life functions: breathing, circulation, temperature regulation, brain activity and cell repair. It is the floor of a person’s daily energy use, before any movement is added.

BMR is expressed in kilocalories per day. It is not a measure of health, and a higher BMR is not inherently better than a lower one. It is a size-and-composition figure. Two people of the same weight can have different basal metabolic rates because muscle tissue is more metabolically active at rest than fat tissue.

Four things move BMR most: body size, body composition, age and sex. Larger bodies use more energy at rest. Bodies with more muscle mass use more energy at rest. Basal metabolic rate generally declines with age, and on average differs between men and women largely because of differences in body composition.

BMR is measured directly by indirect calorimetry, which analyses inhaled and exhaled gases in a controlled clinical setting. Almost nobody encounters that. In practice BMR is estimated using published prediction equations, most commonly the Harris-Benedict equation and the Mifflin-St Jeor equation, which take height, weight, age and sex as inputs. Body composition analysers refine that estimate using measured fat-free mass.

That distinction matters. A BMR figure on a screening report is an estimate produced by a formula, not a measurement taken from your metabolism. Treat it as an approximate baseline for tracking change over time, not as an exact number to build decisions on.

What is metabolic rate, and how does BMR differ from it?

Metabolic rate is the total rate at which the body converts food into energy. BMR is one component of it, and the largest one for most people.

The other components are the energy used to digest and process food, the energy used in deliberate exercise, and the energy used in all the incidental movement of a normal day: walking, standing, fidgeting, climbing stairs. Added together with BMR, these make up total daily energy expenditure, sometimes reported as active metabolic rate.

Screening devices report BMR rather than total energy expenditure because BMR can be estimated from measurable physical inputs. Total daily energy expenditure depends on behaviour, which no kiosk can measure. Any device or app that claims to give you a precise daily calorie requirement is applying an activity multiplier to an estimate, which compounds one approximation with another.

What does BMI mean?

BMI, or body mass index, is a single number calculated from height and weight: weight in kilograms divided by height in metres squared. It was designed in the nineteenth century as a statistical tool for describing populations, and that is still what it does best.

BMI is a screening index, not a body composition measurement. It contains no information about fat, muscle, bone or water. It describes weight relative to height and nothing else.

The World Health Organization publishes standard BMI categories used internationally for population health surveillance. In plain terms, a BMI that appears unremarkable against the standard international scale may carry different implications for a South Asian body. Why BMI works for populations but misleads for individuals

BMI is useful to a health ministry and unreliable to a person. The reason is that it cannot tell the difference between kinds of weight.

Four well-recognised limitations:

  • It does not distinguish muscle from fat. A trained athlete and a sedentary person can share a BMI value while having very different body compositions and very different risk profiles.
  • It says nothing about fat distribution. Fat carried around the abdominal organs behaves differently from fat carried under the skin at the hips. BMI cannot see either.
  • Its risk relationship varies by ethnicity. As noted above, the same BMI value does not carry the same meaning across populations, which is why separate action points are recognised for Asian and South Asian groups.
  • It behaves poorly at the extremes of height and in specific groups. Very short or very tall people, older adults who have lost muscle, pregnant women and growing children are all poorly served by a single adult index.

None of this makes BMI worthless. It makes BMI a first filter. Screening programmes use it because it is free to calculate, requires only a stadiometer and a scale, and reliably identifies a group worth looking at more closely. What it cannot do is close the question for any single person.

What is metabolic age?

Metabolic age is a derived figure that compares a person’s estimated basal metabolic rate against reference values for people of different chronological ages. If the estimated BMR resembles the average for a younger age group, the device reports a lower metabolic age.

Metabolic age is not a clinical measurement and it is not used by clinicians to assess health. There is no standardised definition of it across manufacturers, which means a metabolic age from one device is not comparable with a metabolic age from another.

Its honest value is behavioural. In a corporate wellness programme or a community screening camp, a single intuitive number gets people to engage with a report they would otherwise ignore, and to come back for a second reading. That is a real benefit, and it is a different benefit from clinical accuracy.

What is body age?

Like metabolic age, body age has no agreed clinical definition and no standard formula. Different devices and apps calculate it differently from different inputs, so the figures are not portable between systems.

It is a directional indicator that condenses several measurements into one number for the reader’s benefit, and it loses information in the process. The individual measurements underneath it are more informative than the composite.

Who should have body composition screened?

Body composition screening is general preventive screening, not a test ordered for a specific complaint. It is most useful for people and groups who benefit from a tracked baseline.

  • Adults joining a workplace wellness or annual health programme. A baseline taken once and repeated on a fixed rhythm is far more informative than one reading in isolation.
  • People whose doctor has asked them to monitor weight-related risk markers. Screening supports that monitoring between clinical appointments; it does not replace the appointment.
  • Gym, fitness and nutrition programme members under professional supervision. Composition tracks what a scale cannot, which is whether weight change reflects fat, muscle or fluid.
  • Community and CSR screening camps. Height, weight, BMI and composition are quick, non-invasive and need no consumables, so they work well as an entry point that leads into vitals, blood sugar and blood pressure screening.
  • Older adults being monitored for changes in muscle mass. Loss of muscle with age is clinically meaningful and is invisible on a weighing scale.

Two groups should not rely on these readings. Bioelectrical impedance devices are generally not recommended for people with implanted electronic cardiac devices such as pacemakers, and readings in pregnancy are not interpretable in the usual way. How to read these numbers responsibly

Read the trend, not the reading. Impedance-based composition figures move with hydration, recent meals, recent exercise and even foot contact quality. A single reading taken under unknown conditions carries little information. A series taken under consistent conditions on the same device carries a great deal.

Compare a person only against themselves. Because manufacturers use different formulas, composition and metabolic age figures are not comparable across devices, and cross-device comparison produces false alarms and false reassurance in equal measure.

Do not use any of these numbers to set a diet, a calorie target or a training load on your own.
